Reissue W-2

Your Form W-2 (Employee Wage and Tax Statement) starting with tax year 2012 is available online. A paper Form W-2 form will be mailed out by January 31. If you do not receive this paper form, or need to re-print it for any reason, you may do so by visiting My.IllinoisState Self Service. After using Central Login, you may access the Form W-2 by utilizing the iPeople menu selection 'Self Service' then 'Payroll and Compensation' and 'View W-2/W-2c Forms'.

If you are not able to access the online portal to obtain an electronic copy, then you can submit a W-2 Replacement Request to the University Payroll Office by mail or in person, by fax (309) 438-3357, or by secure form Dropbox Sensitive Form Submission.

Paper versions of replacement Form W-2s can also be obtained by contacting the Payroll Office at (309) 438-7677 or in person at Payroll Suite E in Uptown Crossing.
