Exempt Form W-4 Renewal Procedure

Any Form W-4 claiming EXEMPT from tax withholding will expire on February 15th. To continue as exempt from tax withholding, you must complete a new Form W-4 claiming EXEMPT for the current Calendar Year.

A Form W-4 can be submitted in one of two ways:

• Using the Self-Service functionality within My.IllinoisState
• Submitting a hardcopy to the University Payroll Office. The form can be printed from here.
  • The form can be delivered to the University Payroll Office by mail or in person, by fax (309) 438-3357, or by secure form Dropbox Sensitive Form Submission.

NOTE: Taxes withheld from any payroll processed prior to the receipt of a new W-4 will not be refunded.
