Form W-2

W-2 forms (Wage and Tax Statements) are mailed annually in mid January.

Electronic versions of these forms are available through My.IllinoisState. Address changes made after January 14th will not be included for the W-2 mailing.

For those circumstances where an employee has not received their W-2 form in the mail and cannot access the online portal, to obtain an electronic copy, those employees should submit a W-2 Replacement Request to the Payroll Office. The Payroll Office will start to process these requests beginning on February 14th.

W-2/Pay Advice Reconciliation Template

Employees often ask what amounts are reported on a Form W-2 and why the amounts on their pay advices are different from amounts on their Form W-2. Many of these questions focus on understanding the amounts in the numbered boxes on the Form W-2, followed by:

  • why the amounts reported on their Form W-2 and the amounts on their pay advices are different, and
  • why Federal and State Taxable Wages differ from Social Security and Medicare Wages.
