Form 1095-C for the 2025 tax year is available upon request. Due to a recent change in federal law (Paperwork Burden Reduction Act), Illinois State University is no longer required to automatically mail Form 1095-C to employees. However, we will gladly provide a copy to any employee who requests one. See Form 1095-C Details
